We recognise the value of clinically‑led improvement programmes such as Getting It Right First Time (GIRFT) in identifying and addressing unwarranted variation in care, including in gastroenterology. Rather than being incorporated into the National Quality Strategy, which will set out overarching principles and priorities for improving quality, GIRFT recommendations are developed through detailed clinical engagement, analysis of national data, and close working with professional bodies, providers, and systems. Specialty‑specific recommendations from programmes such as GIRFT are used to inform delivery at national, regional, and local levels, rather than being incorporated as prescriptive elements of the strategy itself.

To ask the Secretary of State for Health and Social Care, what steps his Department is taking to ensure people at risk of disease-related malnutrition are supported through the (a) implementation of the 10 Year Health Plan and (b) provision of neighbourhood health services.
Disease-related malnutrition is a clinical condition affecting all ages across the community and in all health and care settings. Diagnosis and detection are key, and health staff are trained to spot the early warning signs of malnutrition so effective individual treatment can be put in place.
All National Health Services are recommended to adhere to National Institute for Health and Care Excellence clinical guidelines CG3, titled Nutrition support for adults: oral nutrition support, enteral tube feeding and parenteral nutrition, and Quality Standard QS24 which sets out clear requirements for nutritional screening and support across all care settings.
Under these guidelines, all hospital inpatients are screened for malnutrition risk on admission and regularly thereafter. In primary and community care, people are screened when registering at general practice surgeries and when there is clinical concern, and care home residents are screened on admission and when there is a clinical concern. It is also recommended that screening should be considered at other opportunities, for example health checks and flu injections. Screening is undertaken using validated tools such as the Malnutrition Universal Screening Tool (MUST). Healthcare professionals receive training in identifying and managing malnutrition, and individualised care plans are developed for those identified as at risk.
The Department recognises the significant impact of disease-related malnutrition on health outcomes and healthcare costs. The Government’s 10-Year Health Plan places a strong emphasis on prevention and early intervention. It supports the expansion of community-based services, including neighbourhood health centres, which will provide nutrition advice, weight management support, and will address the wider determinants of health. These measures aim to shift care closer to home and reduce reliance on hospital-based services.
We have launched the National Neighbourhood Health Implementation Programme in 43 places across England to improve the care they provide to their communities. Further information on the National Neighbourhood Health Implementation Programme is available at the following link:

To ask the Secretary of State for Health and Social Care, what assessments his Department has made of how to best support patients suffering with overlapping illnesses.
We recognise the growing prevalence and complexity of overlapping and co-existing chronic illnesses and long-term conditions, and the importance of improving diagnosis, management, and support for these patients.
Through the 10-Year Health Plan, we are expanding community diagnostic services, introducing Neighbourhood Health Centres, and deploying multidisciplinary teams to provide holistic support. Patients will benefit from enhanced NHS App functionality, including My Medicines and My Health, and will be able to self-refer to specialist services where appropriate.
Additionally, the 10-Year Health Plan’s commitments on artificial intelligence will give clinicians advanced tools for faster diagnosis, predictive analytics, and personalised care planning. For people with long-term conditions, this means earlier interventions, better monitoring of complex needs, and more time for clinicians to focus on patient-centred care rather than administrative tasks. The plan also commits to 95% of people with complex needs having a personalised care plan by 2027.
The appointment by NHS England of a National Specialty Adviser on multi-morbidity provides expert leadership to improve care for people with multiple long-term conditions, ensuring services are better coordinated, evidence-based, and focused on holistic patient needs.
We also recognise that doctors can find it challenging to diagnose Ehlers-Danlos syndrome (EDS) and postural tachycardia syndrome (PoTS) because these conditions share symptoms with many other disorders.
Resources such as the Royal College of General Practitioners’ Syncope Toolkit for PoTS and the EDS Toolkit, now maintained by Ehlers-Danlos Support UK, are improving clinician awareness of both conditions. The National Institute for Care Excellence provides a clinical knowledge summary on blackouts and syncope to support consistent assessment and diagnosis of PoTS. NHS England commissions a National Diagnostic Service for rare EDS subtypes.
