Chris Grayling: To ask the Secretary of State for Health what the total cost of PFI-based capital investment in the NHS in London has been since 1997; and what revenue costs were incurred on an annual basis to service the financing facilities. [133747] Mr. Hutton: For private finance initiative (PFI) schemes with a capital value greater than £10 million, the total capital cost of PFI-based investment in the four new London strategic health authorities since 1997 is £975,900,000. The capital value of PFI schemes is defined as: total capital cost to the private sector including the costs of land, construction, equipment and professional fees but excluding value added tax, rolled up interest and financing costs. As PFI procures a service, rather than the underlying asset, capital values are necessarily estimates. Unitary payments on a PFI scheme--paid for from a trust's general revenue allocations--include elements for `hard' and `soft' facilities management services as well as financing costs. A partial breakdown of the unitary costs for schemes with a capital value exceeding £20 million is held centrally. This shows the split between `availability' and `services' where `availability' includes costs attributable to `hard' facilities management and financing, but does not distinguish further. To collect this information would mean reviewing the financial models for all these schemes which would incur disproportionate costs. Unitary payments on PFI schemes become payable when the scheme becomes operational. The first to do so was Queen Mary's hospital, Sidcup, which took its first patient at the end of March 2000; the most recent to open was St. George's in September 2003. Chris Grayling: To ask the Secretary of State for Health if he will make a statement on the pilot of the "Forecasting of the Nation's Health Service"; if he will list the trusts which participated in the pilot of the "Forecasting of the Nation's Health Service"; and what plans he has to expand the pilots of the "Forecasting of the Nation's Health Service" to further trusts. [133930] Ms Rosie Winterton: To support national health service managers in anticipating circumstances of weather related high demand, the Met Office (in partnership with the Department of Health and NHS pilot sites) ran a weather forecast/health demand trial project, "Forecasting the Nation's Health", supported by Her Majesty's Treasury "Invest to Save" funding over the winters of 2001-02 and 2002-03. The aim was to provide NHS managers with advance warning of whether to expect increased or reduced emergency admissions and allow them to plan the use of resources more efficiently. This work was without precedent internationally. The trial project has been completed and two external evaluations examined the reliability and value of such workload forecasting for the NHS. NHS users have been closely involved in the evaluation. Chris Grayling: To ask the Secretary of State for Health what action (a) has been taken and (b) is planned to address NHS provision for the management of overweight and obese patients that were identified as patchy in the National Audit Office report on Tackling Obesity in England. [127688] Miss Melanie Johnson: The NHS Plan and the national service framework for coronary heart disease (CHD) (launched after the National Audit Office survey), sets standards for health authorities in partnership with local authorities, primary care trusts (PCTs) and NHS trusts, for the development and delivery of effective policies for reducing overweight and obesity, promoting healthy eating and increase physical activity. The NHS Priorities and Planning Framework for 2003-06 includes targets on reducing CHD. One of these targets requires practice-based registers and systematic treatment regimes, including appropriate advice on diet, physical activity and smoking, to cover the majority of patients at high risk of coronary heart disease, particularly those with hypertension, diabetes and a body mass index greater than 30 (that is, people who are obese). PCTs, as local providers and commissioners of services, determine the most appropriate methods to deliver health care to their populations, based on clinical need and effectiveness, as well as local circumstances, drawing on the available evidence. The Health Development Agency will publish their review of effective interventions for the prevention and treatment of obesity later this year. Work on how to put the evidence into practice is also being taken forward. In line with recommendations of the National Audit Office and Public Accounts Committee reports, the National Institute for Clinical Excellence and the Health Development Agency have been tasked by the Department of Health to work collaboratively to develop guidance on the identification, prevention and management of obesity and maintenance of weight reduction.

Chris Grayling: To ask the Secretary of State for Health (1) how many hospitals have a specialist stroke unit; and what percentage of hospitals this represents; [127704] (2) what progress has been made towards the target of 100 per cent. of hospitals having a specialist stroke unit by April 2004. [127705] Dr. Ladyman: The most comprehensive source of information is the National Sentinel Stroke Audit, carried out by the Royal College of Physicians. The audit was commissioned by the national health service in 1998 and was last updated in 2002. The results can be found at http://www.rcplondon.ac.uk/pubs/ strokeaudit01-02.pdf. The 2002 audit shows that 73 per cent. of trusts who participated had a stroke unit compared to 56 per cent. three years earlier. The audit shows that 80 per cent. of trusts that participated have a consultant physician with specialist knowledge of stroke who is formally recognised as having principal responsibility for stroke services. While it is recognised that some services need to increase their capacity, the audit notes that very significant improvements have already been made. Our major vehicle for further improving standards for stroke services is through the older people's national service framework (NSF) which sets specific milestones for improvement by 2004 of stroke services in primary care trusts, specialist services and general hospitals that care for people suffering from a stroke. Our document, "Improvement, Expansion and Reform", which sets for the NHS a Priorities and Planning Framework for 2003-06, makes clear that implementation of the older people's NSF is a top priority, and that the 2004 milestone around specialist stroke services is a key target. We monitor progress against the key milestones in the NSF. From the information gathered so far we know that 83 per cent. of the hospitals who have replied, now have plans to have a specialist stroke service in place by April 2004. Implementation of the NSF for older people is leading to real improvements in stroke services and making a difference to people's lives. The development of better services and improved standards in stroke care requires the right balance between national standards, provided by the NSF, national clinical guidelines and local control. It will be important to ensure that stroke care services are co-ordinated and fully integrated within a full range of other relevant local services.
