It is the responsibility of primary care trusts to make decisions on the commissioning of lymphoedema services based on guidance already issued by the National Institute for Health and Clinical Excellence (NICE) on treatment for lymphoedema. In 2002, NICE updated its "Improving outcomes in breast cancer" guidance, which makes recommendations on the identification and management of lymphoedema and on the provision of lymphoedema services. In 2004, NICE made further recommendations for commissioners of health services on the provision of the range and volume of rehabilitation services, including lymphoedema therapists, in its "Improving Supportive and Palliative care for Adults with Cancer" guidance. Recommendations on lymphoedema management were also made in NICE'S clinical guidelines on breast cancer published in February 2009. This guidance can be found on NICE's website at:"www.nice.org.uk."Through the Department's National Cancer Survivorship Initiative, formally launched in September 2008, we are looking at the late effects and long-term consequences of cancer and its treatment, including effects such as lymphoedema.

The Department has commissioned a number of surveys and pieces of research on use of the European Health Insurance Card (EHIC). Most recently, this informed departmental press and media communications to make the British population aware that a new EHIC is only valid for five years. All British citizens should make sure they have a valid EHIC so they can access state-provided health care that becomes necessary while travelling in Europe. Travel insurance is also needed as the EHIC does not cover all eventualities (like repatriation).

The Government gave their response to Lord Crisp's report in March 2008. Since then the Department has been working with the Department of International Development to implement several recommendations from the Crisp report. This includes:"an independent evaluation of healthcare links between the United Kingdom and developing countries, completed in May 2008;""a Links Centre to help signpost, advise and develop healthcare links, operational from May 2009;""a Funding scheme that will provide some £1.25 million per year over three years, also operating from May 2009; and""a Department of Health Framework for International Development. This will provide greater clarity on how individuals, NHS and allied agencies can best maximise their potential through healthcare links with developing countries. We are holding a series of consultation events over the summer for publication in October 2009."

I know that the hon. Lady follows these considerations very closely, and I commend her for that. She will know that the Government have committed extra finance, to a total of some £340 million over this spending cycle, through both the Department for Children, Schools and Families and the Department of Health. We have also made clear the future priorities for both the operating framework and the present structure. First, we need to address the issues of palliative care and short breaks. Secondly, we need to look at the clinical pathways and put in place individual care plans. That addresses precisely the point that the hon. Lady made about health visitors, for instance, and the complex needs of these young people. Finally, we must address the question of managing medicines in schools. Those three sets of priorities have been identified by parents, people in the voluntary sector and the young people themselves. [Interruption.] If the hon. Member for South Cambridgeshire (Mr. Lansley) would just stop talking from a sedentary position, I would be able to answer the question. The day that the Conservative party provides answers to anything will truly be a damascene conversion.
I return to the final point made by the hon. Member for Congleton (Ann Winterton) about health visitors. I can confirm that the work being taken forward by the Government in the spring of this year will concentrate on the valuable role that health visitors play in meeting the complex needs of these young people.

I understand that the numbers of health visitors have been cut over the past four years, so when will they be back to 2005 levels? The Government are determined to make progress by accepting recommendations previously made to us, by concentrating specifically on care plans, and by working with GPs on identifying issues, early intervention and providing the correct support. The Minister of State, my hon. Friend the Member for Corby (Phil Hope), has today clearly indicated our determination to make sure that the very highest standards are available to all those who access our health services.

The Secretary of State pledged in his first speech as Health Secretary to meet the care needs of people with a learning disability, but in the light of today's shocking findings by the ombudsman on the NHS's failures in long-term care for young people with learning disabilities, when will the Minister of State and the Secretary of State convert the words of January's strategy—itself an admission of the failure of the Government's 2001 "Valuing People" document—into the action that is so urgently needed if we are to avoid a repeat of the shocking discrimination and damage suffered by those with learning disabilities?

I entirely agree with the points that my hon. Friend makes, which relate particularly to the work being done on end-of-life care, and specifically to the requirements on which primary care trusts have been asked to focus, with regard to short breaks, and palliative and end-of-life care, for very vulnerable young people and children.

Does my right hon. Friend agree that, when we talk about improving the care provided for children with complex care needs, we should focus particularly on respite care and day care, for the parents as well as for the child? The press covers far too many cases of parents whose lives have come to tragic ends because of the enormous stress and strain associated with providing care, over a very long period indeed, for a child with the extensive disabilities that we often see. We need to focus on the parents as well, do we not?

We will also look at his recommendations on wider issues to do with speech, language and communication, and so at the collaboration that needs to take place beyond the health service, particularly through local area agreements, to make sure that we deliver on the recommendations that he rightly made.

One group of children and young people with complex health needs consists of those who require high-tech, expensive communication aids to express their hopes, needs, fears and interests. I welcome the proposed joint commissioning pathfinders, to which the Government have sensibly committed. Will the right hon. Lady confirm that they will be taken forward with all due haste, and that the Government will look at other aspects of joint commissioning, and models for the provision of alternative and augmentative communication for children who are desperately in need of it?

The Department produces an annual updated Heatwave Plan which includes steps to reduce the effects of high temperatures in summer, especially for those groups who's health is most likely to be affected, including the very young, the elderly and other vulnerable groups. This guidance is disseminated to health and social care organisations and local authorities. Additionally, there is a public facing information leaflet on how individuals can protect themselves in hot weather.

The Department has had no discussions with the Thalidomide Trust on health care for people affected by Thalidomide.The trust has declined a recent offer of a meeting with Department officials to discuss health and social care support for those affected by Thalidomide.

It is the responsibility of the national health service to deal with incidences of vitamin D deficiency in their local area as this is a direct health care matter. The Department recommends a daily supplement of vitamin D to all pregnant and breastfeeding mothers, infants, young children, older people and those who do not expose their skin to sunlight who are particularly at risk of deficiency. The Department is developing an information leaflet to reiterate the importance of vitamin D to all health professionals and supports the provision of vitamin supplements under the Healthy Start scheme.

It is the responsibility of primary care trusts to ensure that their existing health care staff are well trained and provided with the opportunity to receive continuing professional development.However, the Government are committed to actively supporting the training of undergraduate doctors, some of whom will go on to become general practitioners, in helping to reduce the harms that are attributable to alcohol misuse.This will include ensuring that around 60,000 new doctors leaving medical training in England, over a 10 year period, will be able to deliver competent practice in both drug and alcohol misuse, including a clear focus on the recognition and the management of risky and harmful alcohol consumption.We allocated £650,000 in 2008 for the developmental work medical schools will need to carry out, to embed such an integrated substance misuse curriculum into their own core teaching and training programmes. This funding will enable the first major tranche of medical schools to make the changes needed; with full roll-out across all schools over the following three years.
