Dr. Fox: To ask the Secretary of State for Health if he will set out the formula and (b) calculations on which the NHS reference costs are based. [134951] Mr. Hutton: The Reference Costs collection entails collecting unit cost and activity data from all National Health Service providers in England, for each financial year. Various appropriate activity currencies are used for a range of services and treatments provided by the NHS, which form part of the scope of the Reference Costs collection. Unit cost data is produced by submitting NHS organisations in accordance with the rules detailed in the NHS Costing Manual, to ensure that both cost and activity data are calculated on a consistent and comparable basis. The Department of Health collates such data, and produces national average unit cost figures, by calculating, for each service/treatment area, the total national cost for the service, and then dividing this by the total reported activity for that service, at national level. All national average costs are published in the National Schedule of Reference Costs, at service level, in conjunction with national activity levels, and interquartile ranges, detailing the ranges between which the middle 50 per cent. of NHS provider costs lie. To calculate National Reference Cost Index scores at individual organisation level, the individual NHS providers' activity is multiplied by the national average unit costs for each service/treatment category, undertaken by that NHS provider. This produces a total expected cost for each service/treatment area for each provider. When added together, this establishes an expected cost that each provider would be expected to incur if they were operating at the national average cost per treatment, procedure, etc. An Index score of 100 signifies that on average, a provider is operating at national average cost levels. A score higher than 100 indicates that for their current level of activity, on average, an NHS provider is operating at costs higher than would be expected for this level of activity. Conversely, a score of less than 100 indicates that the organisation is operating at costs less than would be expected, were they to be operating at national average unit cost levels, for a particular financial year.

Dan Norris: To ask the Secretary of State for Health how much is being spent per person in Bath and North East Somerset on all local health services in 2003; and what was spent in 1997. [136328] Ms Rosie Winterton: Bath and North East Somerset Primary Care Trust (PCT) did not come into existence until 2001-02, so figures are provided for the Somerset Health Authority (HA) area for 1996-97 and for the Bath and North East Somerset PCT area for 2002-03. Expenditure is the total expenditure by the HA and the commissioner net operating costs of the PCT. Expenditure per head is normally expressed by weighted head of population, but this is not possible for 2002-03, because weighted population data are not available for PCT areas. In this case, resident population figures have been used. Total expenditure per head _________________________________________________________________________________________________. £ 1996-97 Somerset HA area 608.58 2002-03 Bath and North East Somerset PCT area 821.14 _________________________________________________________________________________________________. Notes: 1. In many HAs there are factors which distort the expenditure per head. These include: the HA acting in a lead capacity to commission healthcare or fund training on behalf of other health bodies; and asset revaluations in NHS trusts being funded through health authorities or primary care trusts. For these reasons, expenditure per head cannot be compared reliably between HAs. 2. Allocations per weighted head of population provide a much more reliable measure to identify differences between funding of HAs. 3. Expenditure is taken from audited health HA accounts and PCT summarisation schedules, which are prepared on a resource basis and therefore differ from cash allocations in the year. The expenditure is the total expenditure by the Somerset HA for 1997 and the net operating commissioner costs of the Bath and North East Somerset PCT for 2002-03. The majority of general dental services expenditure is not included in HA accounts or PCT summarisation schedules and is separately accounted for by the Dental Practice Board. An element of pharmaceutical services expenditure is accounted for by the Prescription Pricing Authority and not by HAs or PCTs. Sources: 1. Somerset HA audited annual accounts 1996-97. 2. Chris Grayling: To ask the Secretary of State for Health what the total cost of PFI-based capital investment in the NHS in London has been since 1997; and what revenue costs were incurred on an annual basis to service the financing facilities. [133747] Mr. Hutton: For private finance initiative (PFI) schemes with a capital value greater than £10 million, the total capital cost of PFI-based investment in the four new London strategic health authorities since 1997 is £975,900,000. The capital value of PFI schemes is defined as: total capital cost to the private sector including the costs of land, construction, equipment and professional fees but excluding value added tax, rolled up interest and financing costs. As PFI procures a service, rather than the underlying asset, capital values are necessarily estimates. Unitary payments on a PFI scheme--paid for from a trust's general revenue allocations--include elements for `hard' and `soft' facilities management services as well as financing costs. A partial breakdown of the unitary costs for schemes with a capital value exceeding £20 million is held centrally. This shows the split between `availability' and `services' where `availability' includes costs attributable to `hard' facilities management and financing, but does not distinguish further. To collect this information would mean reviewing the financial models for all these schemes which would incur disproportionate costs. Unitary payments on PFI schemes become payable when the scheme becomes operational. The first to do so was Queen Mary's hospital, Sidcup, which took its first patient at the end of March 2000; the most recent to open was St. George's in September 2003.
