There is a range of initiatives the department has taken or is taking in this area including: there are a number of National Institute for Health and Clinical Excellence quality standards commissioned by the department that are either in place or are being developed for a range of conditions and pathways affecting older people including incontinence, nutrition support in adults, patient experience in adult National Health Service services, delirium, dementia, osteoarthritis and falls in a care setting; one of the eight high-impact actions developed for nursing, relates to keeping people nourished and getting better and aims to stop unintentional weight loss and dehydration in patients. This action is taken locally with initiatives such as red trays to identify patients who need help with eating and drinking and better management of charts. Since April 2010, it has been a requirement of a hospital's continuing registration with the Care Quality Commission (CQC), that service users are protected from the risks of dehydration and inadequate nutrition; within the NHS Operating Framework and the NHS Outcomes Framework, there are a number of indicators to support better care for older people. These include dementia care, hospital-acquired infections, emergency readmission rates, improving recovery from fragility fractures and helping older people to recover their independence after illness or injury; and the department is also funding national audits of falls and bone health, dementia, continence and hip fractures. Local involvement networks in each local authority area are able to enter and view services to obtain the views and experiences of patients, service users and their families about their experiences of care and treatment at the time they are receiving it. Subject to the passage of the Health and Social Care Bill, local HealthWatch organisations will continue this enter and view activity. This information can be shared with the CQC's compliance inspectors, which will help to focus the inspection on specific concerns and thereby help the CQC to hold local services to account. Local Healthwatch will also be the champion for public and patients, service users and carers in health and social care services. Healthwatch England will be able to use information from local Healthwatch, and elsewhere, to form a national picture of services. Locally and nationally, Healthwatch will have a remit to make recommendations about how services could or should be improved. The department supports the partnership on dignity in care, which has been established by the NHS Confederation, Age UK and the Local Government Group. The partnership aims to help drive improvements in the levels of care provided to older people in hospitals and care homes and has an independent commission which is currently taking evidence. The department has commissioned a further round of CQC dignity and nutrition inspections for older people in hospital and care home settings.

Information on numbers of deaths from infections contracted in hospital is not available. Death certificates rarely specify the place where an infection was acquired. Death certificates do, however, record the place of death. The Office for National Statistics publishes analyses of deaths in England and Wales with meticillin resistant staphylococcus aureus (MRSA) or clostridium difficile (C. difficile) mentioned on the death certificate. The following tables present the number of death certificates mentioning MRSA from 1999-2005 and C. difficile in 1999 and from 2001-2005 by place of death. Information on C. difficile is not available for 2000 (please see note 3).

Incidence figures for prostate cancer are not collected based on how aggressive the cancer is or on the ethnicity of the patient. However, latest figures show that 29,406 men were diagnosed with prostate cancer in England in 2004 and the table below breaks this down by primary care trust (PCT).

Data from the United States had suggested that there was a link between ethnicity and prostate cancer, with black men more likely to develop prostate cancer. To investigate whether this was the same for England, we funded the PROCESS study (prostate cancer in ethnic subgroup) in 2002. This study has now been completed and shows that black men are three times more likely to develop prostate cancer compared with white men, but that they do not have worse outcomes. Follow-up studies are currently being devised and will help to advise how best to take this issue forward.

My Lords, I am grateful to my noble friend. Is he aware that 1,142 haemophilia patients have now died from being infected with HIV and hepatitis C by contaminated National Health Service blood and blood products, making this its worst ever treatment disaster? My noble friend told me on 11 December 2003 that Ministers,"““do not consider that a public inquiry is justified””.—[Official Report, 11/12/03; col. 937.]" Is that still their position, despite mounting concern about the handling by in-house inquiries of the important issues raised—as former health Ministers—by the noble Lords, Lord Jenkin and Lord Owen? Can my noble friend say when the appeal system for the Skipton Fund will be operational; and is he aware that, for it to have been ministerially decreed to deny hepatitis C widows the financial help available to HIV widows, is widely seen as unjust and morally indefensible?

My Lords, is my noble friend aware that contaminated NHS blood products have now caused the deaths of 1,137 haemophilia patients, 896 after being infected with HIV and 241 with hepatitis C, making this much the worst treatment disaster in the history of the National Health Service? My noble friend Lord Winston, vice-president of the Haemophilia Society, has said of HIV and hepatitis C: "The cause is the same, a virus, and it comes from the same source, blood products".—[Official Report, 5/6/98; col. 672.] For the wives of those who have died the result too is the same: devastated lives and widowhood. Why, then, by ministerial decree, are hepatitis C widows denied financial help available to HIV widows? What social justice or morality is there in denying parity of treatment to widows in identically the same tragic position?
