NHS England compiles and publishes monthly delayed transfers of care data. The data is available by each local authority and National Health Service trust, and includes the reason for the delay against a defined set of 10 categories. The data set is published in an online-only, interactive format.
Delays have multiple causes and as a result there is variation between local authorities. A wide-ranging package of support is in place to help areas improve transfers out of hospital and reduce delays, including work as part of Accident and Emergency Improvement Plans to understand and address delays in hospital discharge and promote good practice.
The NHS and local government have also developed new resources, including a high impact change model for reducing delayed transfers of care. In addition, the Emergency Care Improvement Programme provides tailored support to local systems to support rapid and sustained improvement in performance.

Adult social care is largely delivered through an independent sector of care provider organisations that operate in a competitive market. As with any market, some providers enter and exit, which is an important mechanism for driving up quality and effectiveness. Market exits are regular occurrences and are handled effectively by local government.
It is of course vital that vulnerable people with care needs do not have their services interrupted if their care provider failed financially and services stop. The Care Act 2014 introduced new duties on local authorities and the Care Quality Commission (CQC) to protect and reassure people with care needs, their families and friends. The Act places a duty on all local authorities in England to temporarily step in and make sure all people in their area continue to have their needs met, regardless of who pays for their care. Recognising that local authorities might struggle if a significant provider were to fail, the CQC has a new function to oversee and monitor the financial sustainability of the largest and most difficult to replace providers. The oversight function provides an early warning to relevant local authorities in the event that one of these providers is likely to fail and their services cease.
Four Seasons Health Care is one of the providers that because of their size, is part of the CQC’s Market Oversight Scheme. CQC continues to monitor the finances of all of the providers in the scheme. Government has no powers to require a private company that is acting lawfully to change its financial approach.
The Care Act 2014 also places new duties on local authorities to promote their local market to ensure all service users have a choice of high quality services available.

The involvement of patients and their carers is central to timely and appropriate discharge from hospital. Each hospital’s discharge policy should set out how patients and carers will be involved and discharge arrangements discussed.
Discussions should include providing full information about the options available and supplying patients and carers with a discharge plan.

The forthcoming carers strategy will reiterate the importance of ensuring that families and carers are involved in the planning of hospital discharge and the role of local partners in ensuring this works well in practice.
The strategy will also highlight the range of guidance and support available to local areas and to families.

We welcome trusts piloting schemes whereby they ask National Health Service patients for two forms of identification to demonstrate an individual’s residency status. This is good practice and helps establish, in a non-discriminatory way, who may be eligible for free NHS care and who may not. We will monitor the impact that identification checks have, including the effect on costs identified and recovered from people not eligible for free NHS care.

To ask Her Majesty’s Government what is the cost of bed-blocking in NHS hospitals in each of the past three years and what plans they have to invest in social care services to reduce those costs.
To date, the Department has made no formal estimate of the costs of delayed discharge to the National Health Service. However, the Department and NHS Improvement are working together to implement Lord Carter’s recommendations on hospital productivity. As part of this, the two organisations are working with providers to develop a richer dataset around all aspects of the patient pathway, including estimates of the cost of delayed discharge.
At the same time, the Department continues to work closely with the NHS and local government to help local areas improve transfers out of hospital, share best practice, and reduce unnecessary delays. We are funding the NHS’s own plan for the future with £10 billion and we are giving local authorities access to up to £3.5 billion of new support for adult social care by 2019/20.
Since April 2015, the Government’s £5.3 billion Better Care Fund has provided much needed investment in better integrated care through locally developed plans and by putting resources where the local NHS and social services think they are needed. Alongside this we are working with local areas to improve the transfer of patients back into the communities.
In 2016-17, there is a new Better Care Fund requirement on local areas to develop a clear, focused action plan for managing delays, including locally agreed targets. The requirement is designed to reduce delays across the health and care system.
