[5695] Yvette Cooper: [holding answer 20 July 2001]: I apologise to the hon. Member for the delay in responding to this question. I refer the hon. Member to the reply that my right hon. Friend the Secretary of State gave my hon. Friend the Member for Blackpool, North and Fleetwood (Mrs. Humble) on 5 March 2002, Official Report, column 192W. Batches 3741 and 3732 of Diphtheria, Tetanus and Pertussis (DTP) vaccine are believed to have been manufactured and distributed in Eire in the late 1960s and early 1970s. At that time DTP vaccine was not provided by the Department but was either purchased by local health authorities or obtained by general practitioners (GPs) from local retail pharmacists. Thus, information on whether batches 3741 and 3732 were supplied to United Kingdom doctors is not held centrally. Should such vaccines have been issued, the only information on who might have received them will depend on GPs or local health authority clinics maintaining records that will be now over 30-years-old. The Department has asked the then manufacturer's present owners, GlaxoSmithKline, to investigate their records. The National Institute for Biological Standards and Control has also been asked to investigate whether they tested these vaccines. Extensive investigation of the safety of pertussis vaccines was undertaken during the 1970s and early 1980s following negative publicity about brain damage following pertussis immunisation. In 1981 a major report was published by the Committee on Safety of Medicines and the Joint Committee on Vaccination and Immunisation which investigated pertussis vaccine, including suspected adverse reactions to the vaccine. The report of this investigation was published in 1981 ("Whooping Cough", HMSO, ISBN 0 11 3207646) and advises on page 47 that "From 1964 onwards, all vaccines used in the UK also conformed to World Health Organisation standards of preparation and potency". The Yellow Card database has been interrogated for reports of suspected adverse reactions to the vaccine batches 3714 and 3723 which were available between 1968 and 1971. No such reports have been identified. For reports received in the late 1960s and early 1970s, full data are not available for all reports. For those reports where full information is available, the batch number of the suspected vaccine is not always provided. The Yellow Card used at that time did not specifically ask reporters to provide information on batch numbers. The Yellow Card used currently asks specifically for information on batch and since 1992, where this information has been provided by the reporter, there has been full electronic capture of this information on the database. Investigation of reports of suspected adverse reactions following pertussis containing vaccines (principally DTP vaccine) reveals no rise in reactions during the period when, if they were used in the UK, batches 3741 and 3732 might have been given (late 1960s and early 1970s) (see table). It should be noted that a report of a suspected adverse reaction does not necessarily mean that it was caused by the vaccine. Many vaccines are administered in combination and in these circumstances it may be difficult to ascribe the cause of the reaction to an individual vaccine. Adverse reporting rates are influenced by many factors, including the extent of use of a drug or vaccine and any publicity about the vaccine.

Dr. Starkey: To ask the Secretary of State for Health what research is being undertaken to investigate the effects of thiomersal in vaccines on children and links with autism. [1408] Yvette Cooper: The Department is discussing with the Public Health Laboratory Service what suitable research in this area may be possible. The United States Centers for Disease Control have conducted two studies which have not demonstrated a causal association between autism and thiomersal (the amount of thiomersal in vaccines used in the UK childhood immunisation programme is considerably less than that used in the United States programme). A full joint statement by the American Academy of Family Physicians, the American Academy of Pediatrics and the Advisory Committee on Immunisation Practices and the US Public Health Services is available on the world wide web at http:// www.cdc.gov/nip/vacsafe/concerns/thimerosal/joint- statement-00.htm. The Joint Committee on Vaccination and Immunisation, the Department of Health's statutory independent advisory committee, has reviewed the available evidence and has concluded that the present evidence did not support a link between thiomersal in vaccines and autism. They also concluded that there was substantial evidence that the measles, mumps and rubella vaccine--which is a live attenuated vaccine and therefore has never contained thiomersal--is not a factor, but the cause of autism remains unknown. The Department has asked the Medical Research Council to review current knowledge about autism and to suggest possible areas for further research development so that a cause may be found.

Mr. O'Hara: To ask the Secretary of State for Health (1) how many cases of epilepsy have been recorded between 1 November 1999 and 28 February 2001 related to yellow cards submitted in connection with meningitis C injections; [157552] (2) what research is being conducted into the incidence of cases of epilepsy which may be related to meningitis C injections. [157549] Yvette Cooper: On 1 November 1999, a national immunisation campaign commenced to vaccinate all children under 18 years with one of the new meningococcal C conjugate vaccines. The campaign finished at the end of January 2001. As of 16 February 2001, 18.5 million doses of meningitis C vaccine had been distributed. The Medicines Control Agency and the Committee on Safety of Medicine (CSM) has received three Yellow Card reports of suspected epilepsy following vaccination with the meningococcal C conjugate vaccines. A total of 291 reports of suspected seizures have been reported in association with the meningococcal C conjugate vaccines. CSM has concluded that there have been very rare reports of seizures following meningitis C vaccination and individuals have usually rapidly recovered. A significant number of the reported seizures are thought to have been faints. A report of a particular suspected adverse drug reaction does not necessarily mean that it has been caused by the drug. The Yellow Card data can not be used to calculate the incidence rate of a particular suspected adverse reaction, nor can it be used to measure changes in the rate against time. The Medical Research Council has research programmes looking into epilepsy and meningitis, but no program that is examining the incidence of epilepsy after meningococcal C conjugate vaccination.

Mr. Ian Stewart: To ask the Secretary of State for Health how many adverse reactions to the meningitis vaccine were reported, broken down by degree of severity, in each of the last five years. [144510] Yvette Cooper: On 1 November 1999 a national immunisation campaign started, which aimed to vaccinate all children under the age of 18 years with the new Meningococcal C conjugate vaccine by the end of 2000. Approximately 18 million doses of Meningococcal C vaccine have been distributed to date. This should be taken into account when considering the numbers of reports and reactions. The numbers of reports and numbers of suspected reactions following the Meningococcal C vaccines reported to the Medicines Control Agency through the yellow card scheme in 1999 and 2000 are in the table. _________________________________________________________________________________________________. 1999 2000 Total number of reports 2,099 9,264 Total number of suspected reactions 4,592 19,177 Number of serious reports 485 2,326 Number of serious suspected reactions 574 2,676 _________________________________________________________________________________________________. The number of suspected reactions is greater than the number of reports because many reports list more than one reaction, e.g. nausea and headache. A reaction is classified by the MCA as serious if it is a disease, disorder, symptom or sign which is generally recognised as serious clinically or if it could be indicative of a serious disorder. The interpretation of this definition is deliberately broad; reactions such as fainting and swollen gland are classified as serious. Evaluation of safety data from spontaneous suspected adverse reaction reports in the context of reduction of morbidity and mortality from meningitis C, indicates that the overall risk-benefit balance of this new vaccine is overwhelmingly favourable.

Mr. Amess: To ask the Secretary of State for Health (1) if he will publish the medical research and evidence on which the Committee for the Safety of Medicines based their recommendation that levonorgestrel 0.75 mg could be supplied as a pharmacy medication; [129410] (2) what trials have been conducted in the United Kingdom to test the safety and long-term health effects of levonorgestrel 0.75 mg. [129409] Yvette Cooper: A product containing levonorgestrel 0.75 mg has been authorised in the United Kingdom as a prescription only medicine. This followed advice from the Committee on Safety of Medicines (CSM) who were satisfied as to the product's quality, safety and efficacy. The evidence considered included two World Health Organisation sponsored pivotal studies. One study involving approximately 3,000 women in 14 countries, including the UK, has been published in the Lancet (1998). The other, published in Human Reproduction (1993), involved 880 women. Copies are available in the Library. Other supporting data not in the public domain are confidential to the application. Evidence of safety as stated was taken into account when the CSM considered the proposal for levonorgestrel 0.75 mg to be available without prescription. There is also evidence from a study of the effects of self-administering emergency contraception (New England Journal of Medicine, 1998). Levonorgestrel 0.75 mg has been available from pharmacies in France without a doctor's prescription since June 1999. The criteria for prescription control are defined in the legislation. The CSM considered each of these criteria and advised that they did not apply to levonorgestrel 0.75 mg for emergency contraception in women of 16 and over. They considered that all the steps required to supply emergency contraception safely could be successfully completed in a pharmacy.
