Nursing ratios are a matter for local clinical judgment. The exact nurse-to-patient ratio will depend on the number and criticality of the infants on the unit. The British Association of Perinatal Medicine recommends a nurse-to-infant ratio of one to one for neonatal intensive care, one to two for high-dependency care and one to four for special care.

The National Health Service data dictionary does not give a definition for a bay. Its smallest unit is a ward. However, there have been several statements that move towards a working description. The firstof these was in 1997, when the Department ofHealth issued guidance on eliminating mixed-sex accommodation. This stated: "““at a point where a ward has been divided into separate areas to segregate the sexes, there should be screening which is adequate to ensure privacy is maintained, eg which will prevent patients being overheard and overseen by others where this is not desirable””."
In 2002, NHS Estates issued Enhancing Privacy and Dignity—Achieving Single Sex Accommodation. This offered a number of design solutions using various arrangements of bays, single rooms and open areas, and demonstrated the wide range of acceptable solutions.
Also in 2002, in response to a Parliamentary Question, the following advice was given on how wards could be separated into bays: "““Partitioning can be used to divide wards into bays as a means of achieving an acceptable method of protecting patients’ privacy and dignity. To ensure that they provide adequate physical separation, sound reduction and visual privacy the partition must be permanent, rigid and fixed to the building structure. A mobile privacy screen alone is not an acceptable solution to enhancing privacy and dignity for the patient””."

The ““GP Patient Survey—Your Doctor, Your Experience, Your Say”” will be sent to more than 5 million adults in January 2007. The introduction of this survey forms part of the changes introduced into the new primary medical care contract in April 2006 and will deliver the commitment in the White Paper ““Our health, our care, our say. a new direction for community services””, published in January 2006. It will provide a nationally consistent tool for asking patients their views on accessing primary care services in their General Practitioner practice.The survey will be the biggest ever of its kind. The Department has worked closely with the general practices committee of the British Medical Association and Ipsos MORI, the Survey provider, to ensure patient confidentiality is protected.The results of the initial survey will be used by Primary Care Trusts to reward general practices on their achievement against national standards they have aspired to within the improved access directed enhanced service (DES) for 20060-07. Practice participation in the DES is voluntary and the Department does not collect information centrally on the numbers of practices who have done so. Any practice which is participating in the DES will however need to take part in the patient survey if it is to receive the full payment to which it may be entitled under the DES. To date 265 practices (3 per cent.) have confirmed that they are not taking part.

It is not beyond the bounds of possibility that putting some very vulnerable elderly women in mixed-sex wards does not just cause them embarrassment but might put them in real danger of abuse or exploitation.

My Lords, it is down to local trusts, which admit people to hospitals, to be concerned about the safety of patients. They must take all due measures to do that, and the evidence is that where there have been unsatisfactory cases, they have been very rare. It is inevitable that in an emergency, the need to treat and admit will always take priority. In general, patients and the public agree with this.

My Lords, the noble Baroness makes an important point. On 14 November, my colleague Ivan Lewis launched the first-ever national dignity in care campaign. It aims to create a care system where there is zero tolerance of abuse of, and disrespect towards, older people, and a situation where people are as outraged by the abuse of parents and grandparents as they are by that of children.

I repeated the target that we have set, which was largely based on the work done by the Conservative Government on their objectives, definitions and exemptions. We have made good progress in implementing that target, but there will always be places, in particular admission wards and intensive care units, where it will be impossible, in all probability, to remove such facilities. I see that a number of noble Lords opposite are shaking their heads, but I will at a suitable point send them the guidance that was issued by their party.

My Lords, I thank the noble Lord for that reply. Is he aware that I was in a mixed ward? It was just as embarrassing for the men as it was for me. Why should we have any mixed wards left in this country when France and Germany do not have any?
