Over 600 responses were received during the web-based consultation on the new research strategy, of which 21 identified themselves as business/industry, but no further detail is available.The development of new treatments, vaccines and technologies for (a) malaria, (b) tuberculosis, (c) HIV and AIDS and (d) other diseases were raised by many respondents. Responses to the consultation were analysed anonymously and comments can not be attributed to individuals or organisations.

Mr. Sarwar: To ask the Secretary of State for International Development what steps his Department is taking to expand the use of
proven effective drug combinations for AIDS, tuberculosis and malaria treatment in developing countries. [220771]
Mr. Gareth Thomas: The UK Government are committed to increasing access to medicines in developing countries, including drug
treatment programmes for HIV and AIDS, Tuberculosis (TB) and Malaria. In recent years, there has been considerable progress in
making medicines for the treatment of HIV and AIDS, TB and Malaria more affordable.
The UK has worked with the pharmaceutical industry to encourage differential pricing that provides affordable medicines for
developing country use. This includes recommendations agreed with industry on differential pricing from the UK Working Group on
Increasing Access to Essential Medicines in Developing Countries in November 2002.
The UK supports developing countries to make use of internationally agreed flexibilities in the Trade Related Aspects of
Intellectual Property (TRIPs) agreement, that allow recourse to the use of generic versions of proprietary medicines where there is
a public health need.
The UK has pledged £259 million to the Global Fund for AIDS, TB and Malaria (GFATM). 49 per cent. of GFATM grants go
towards the procurement of drugs and commodities for AIDS, TB and malaria resulting in 130,00 people on HIV treatment, 385,000
treated for TB and 300,000 people treated with effective artemisinin containing treatments (ACTs) for malaria. Additional
commitments for each of the three diseases are set out as follows:
The HIV and AIDS Treatment and Care Policy, published in June 2004, sets out the UK's commitment to increasing access to HIV
treatment in developing countries. The UK has provided £3 million to support the work of the World Health Organisation on HIV
and AIDS, with a strong focus on its role supporting the target of 3 million people on treatment by 2005 (3 by 5). In total,
the UK has committed to spend £1.5 billion on HIV and AIDS from 2005-07, a significant part of which will support increased access
to treatment.
The UK has provided £2 million to the Stop TB Partnership since 2002. The Partnership has established mechanisms, which
support countries to access stable supplies of effective anti-TB medicines.
DFID is the largest funder of the Roll Back Malaria (RBM) partnership, having contributed £48 million to date. The UK is
working with RBM and other partners to secure greater international commitment for affordable pricing of ACTs. DFID Kenya has also
recently awarded a start up grant of £35,000 to support increased cultivation of artemisia annua in Kenya. This is the key raw
material for ACT production and is currently in short supply.
Effective health systems are equally important if all medicines are to be delivered safely. The UK has committed over £1.5 billion
since 1997 to help developing countries strengthen the health and supply systems. Increasing Access to Essential Medicines in
Developing Countries: UK Policy and Plans', published in June 2004, gives further information on UK positions and activities in this
crucial area.

Mr. Gareth Thomas: The UK is the second biggest bilateral donor on AIDS. It is directly active on AIDS in 39 countries. We support
the national AIDS strategies of developing country partners, involving all relevant parts of government. DFID supports a number of
multilateral institutions including WHO, UNAIDS and UNFPA and was a prime mover in setting up the Global Fund to fight AIDS, TB and
Malaria (GFATM), committing US $280 million over seven years.

Mr. Edwards: To ask the Secretary of State, Department for International Development what support the Government can give to tackle
HIV/AIDS in Caribbean countries. [183727]
Mr. Gareth Thomas: DFID has recently agreed a £2.1 million programme of assistance to the Caribbean to firstly support the
Pan Caribbean Partnership on HIV/AIDS. Secondly to support the joint programme of six Eastern Caribbean states to assist in for
example training clinical care teams and the bulk purchase of anti retro viral medicines and testing kits. And third support to the
Caribbean Community Secretariat (CARICOM) to organise a high level Forum in November 2004 that will consider how to reduce stigma
and discrimination against people living with HIV/AIDS.
The UK Government also makes substantial contributions to the programmes of the European Commission, the World Bank and the Global
Fund for AIDS, Tuberculosis and Malaria, all of whom are currently supporting HIV/AIDS programmes in the Caribbean.

Mr. Gareth Thomas: Malaria is a leading cause of mortality and morbidity in many developing countries, limiting economic and social
development. It is understood to be both a disease of poverty and a cause of poverty. Annual economic growth in countries with high
malaria transmission is lower than in countries without malaria and in some African countries it penalises economic growth by up to
1.3 per cent. per year. This leads to substantial differences in GDP between countries and severely restrains the economic
growth of the entire region.
The social impact of malaria includes the loss of earnings, human pain and suffering. It hampers children's social development and
educational opportunities. The disease affects individual and household decision-making in ways that have a negative impact on
economic productivity and growth.
As part of an international effort, the UK is committed to reducing the impact of malaria. We provide support through global
partnerships such as Roll Back Malaria and the Global Fund to Fight AIDS, TB and Malaria as well as supporting country programmes
(projects, support to health sectors and general budget support).

Malaria partnership, and look to it to inform us on the impact of malaria in developing countries.
DFID is in regular contact with health officials in developing countries and provides assistance through its support to developing
country programmes. As part of the international effort, we are working to ensure that the Roll Back Malaria partnership lends its
support to nationally led programmes, processes and strategies, and provides sound evidence-based support for the effective use of
additional resources from the Global Fund to Fight AIDS, TB and Malaria.

Mr. Gareth Thomas: DFID provides support to UNAIDS, and looks to them to provide information on the global status of the HIV/AIDS
epidemic. Last December, UNAIDS launched the Global Epidemic Update (available at www.unaids.org) which provided a synthesis of
trends in the epidemic in all regions. They will be producing detailed country information to launch at the World AIDS Conference in
July 2004 in Bangkok.

Dr. Tonge: To ask the Secretary of State for International Development what plans he has for ensuring the adequacy of healthcare

Mr. Gareth Thomas: DFID is committed to increasing access to treatment as part of a comprehensive response to HIV/AIDS, and welcomes
the WHO/UNAIDS target to get three million people on treatment by 2005. The UK will provide £12.5 million to the World Health
Organisation in 2003-04 to support its work, including `3 by 5'. We have supported WHO to make the strengthening of health services
a key part of their strategy to reach the `3 by 5' target. As treatment programmes are expanded it will be essential to invest in
health systems in order to ensure that services for HIV, including voluntary counselling and testing and the provision of treatment,
and other health services, are of good quality and accessible by poor communities and particularly women. This includes better
co-ordination of public, private and NGO treatment initiatives. We also support community involvement in health service delivery as
a well- informed civil society has been shown to be essential in the sustainability of comprehensive health systems.
DFID has provided £1.5 billion since 1997 to the strengthening of health systems in developing countries and we see this as an
essential element of the response to the HIV/AIDS epidemic. We are working with partners to build sound healthcare systems as the
cornerstone of efforts to accelerate progress to achieve the Millennium Development Goals; to improve maternal and child health and
reduce the impact of HIV and other major communicable diseases. The UK was a prime mover in setting up the High Level Forum on
Health MDGs, which was held in Geneva in January 2004 and at which developing and donor countries discussed concrete actions to
increase health service capacity to meet international development goals. DFID also supports the integration of HIV/AIDS into wider
health sector policies and national poverty reduction strategies.

Mr. Gareth Thomas: The most recent official statistics from Bolivia (1992) put the maternal mortality rate at 390 for every 100,000
live births. This is one of the highest figures in Latin America and is disproportionately high for a country of the economic
status of Bolivia. A key cause of mortality is unwanted pregnancy and subsequently complications resulting from illegal abortion.
The aim of the Post Abortion Care (PAC) Services programme is to help Bolivia progress towards the Millenium Development Goal of a
75% reduction in maternal mortality by 2015. The approach of the programme is to increase access by poor women experiencing
post-abortion complications to high quality, cost-effective Post Abortion Care (PAC) Services under the Basic Health Package. The
programme supports the provision of outpatient PAC with manual vacuum aspiration (MVA) services at five tertiary-level hospitals,
strengthened training capacity for PAC with MVA at five teaching hospitals, the institutionalisation of PAC training for medical
residents, the introduction by Government of Bolivia of the PAC-with-MVA model in secondary-level hospitals in rural areas, and the
strengthening of the supply of PAC-with-MVA medical equipment and drugs, for present and future services.
The programme is managed in partnership with the Government of Bolivia and supports its Sexual and Reproductive Health Programme.
Local communities are closely involved in discussions on the introduction of PAC with MVA in secondary level hospitals, and the
church is consulted in the context of discussion on national reproductive health strategies.

In addition, the Department spent £0.69 million on Contraceptives Supply and Logistics in Bolivia, £0.13 million on the Andean
Reproductive Health Initiative in Bolivia and Peru in 2002-03, £0.11 million on the Strategic Pilot Project in Sexual and
Reproductive Health and £0.21 million on the Bolivia Post Abortion Care Project. These figures do not include any support provided
through contributions to multilateral or non- governmental organisations, which are not broken clown on a sectoral basis.
DFID spent over £270 million on reproductive health and HIV/AIDS programmes in financial year 2002-03. We regard reproductive health
as an essential and inseparable element of good health. We believe that the best way to deliver this is for people to be given the
right, freedom and support necessary to enable them to take full individual and personal control of their own fertility and
reproductive health. Reproductive health continues to be a key priority for DFID and we remain firmly committed to the 1994 Cairo
International Conference on Population and Development (ICPD) target of achieving access to reproductive health for all by 2015.
DFID support for reproductive health is wide ranging. It covers HIV/AIDS programmes, prevention and treatment of sexually
transmitted infections and maternal and adolescent health. Reproductive health also encompasses the services for family planning,
contraception and childbearing that make an important contribution to increasing women's choice and opportunity and to preventing
unplanned or unwanted pregnancy. Family planning services also provide an important opportunity to improve sexual health, and in
particular, to increase women's access to methods that help prevent HIV infection. DFID's total expenditure for 2003-03 cannot
therefore be apportioned between HIV/AIDS and family planning, as many reproductive health programmes and projects contribute to
both objectives (for example, through the provision of information on family planning and HIV/AIDS prevention or the provision of
condoms).

Angus Robertson: To ask the Secretary of State for International Development how much the Department has given to the sexually
transmitted disease programme in the Russian Federation in 2003; what percentage of the Department's total expenditure for
population reproductive health programmes that represents; and if he will make a statement. [131796]
Mr. Gareth Thomas: DFID has supported four projects related to the control of sexually transmitted diseases in the Russian
Federation in financial year 2002-03. A particular priority is the prevention of HIV transmission among vulnerable groups, including
injecting drug users and commercial sex workers.

The European Commission, through the TACIS programme, is supporting two large-scale projects on HIV/AIDS prevention: a) HIV/AIDS
Prevention and Combating: the project allocation is Euro 2,500,000; b) Preventive Health and Education: the project allocation is
Euro 3,000,000.
DFID contributes approximately 20 per cent. of these project budgets through its funding to the European Commission.
DFID spent over £270 million on reproductive health and HIV/AIDS programmes in financial year 2002-03. Spending on projects related
to these issues in the Russian Federation in the same financial year accounted for less than 1 per cent. of the total.
