Consumer contact with Local Authority Trading Standards Services has been handled by the Government funded Consumer Direct telephone and online advice service since June 2004. Before this time contacts were made direct to the 205 local authorities and are not readily available or collected by BERR.Contacts recorded against the categories of hair removal therapy; beauty treatments; cosmetic therapies; hair replacement therapies and the term ““laser”” identifies 199 contacts since June 2004. A contact may be a complaint or could be a request for other information such as pre-shopping advice. Consumer direct does not specifically record contacts regarding non-surgical laser and intense pulsed light treatments.

Progress is being made in the international effort to tackle HIV and AIDS. There has, for example, been a significant scaling up in the level of financial assistance to tackle the epidemic, and the number of people receiving antiretroviral treatment in poor countries has risen from 400,000 to more than 2 million. There is, however, a lot more to do.

The reason why the strategy is forthcoming is that there is still work to do on its preparation, so I cannot give the hon. Gentleman a preview of what it will contain. One of the reasons why we included specific targets when we published our previous AIDS strategy in July 2004 was to generate significant new political momentum behind the effort to fight AIDS in general and the AIDS orphans crisis. I hope that he will recognise, from the research that he has done, that political momentum behind the fight against AIDS has increased significantly and that much greater effort is being put into tackling the specific problems faced by AIDS orphans.

We need to do both. We must ensure not only that we continue to help tackle the HIV/AIDS epidemic, but, as the question from my hon. Friend the Member for Warrington, South (Helen Southworth) indicated, that we do more to tackle a range of other health conditions. We cannot fight AIDS without there being more health workers—more doctors and more nurses—in-country, and we cannot tackle infant and child mortality without there being more health workers in place. We need to do more to tackle the specific problems associated with HIV/AIDS, but we must also ensure that our response to HIV/AIDS helps to tackle those broader health questions.

I can confirm to my hon. Friend that we do not support abstinence-only programmes for HIV prevention, because none of the available evidence suggests that such programmes are an effective strategy for HIV prevention. He raised a point about the valuable contribution that voluntary sector organisations make. I have had the privilege of seeing some of the work that Christian Aid supports in southern Africa, so I take his point about the need for us to continue to work with the voluntary sector. I hope that he will recognise that where we can have confidence in the commitment of Governments to preventing HIV and AIDS, we should continue to help them scale up their ability to tackle AIDS in their countries.

Will the Minister take this opportunity to distance himself from the more weird and wacky groups that are suggesting that abstinence is the only way to combat HIV/AIDS in parts of the world? Will he also take the opportunity to tell the House that as many moneys will go via voluntary organisations and non-governmental organisations as will go through some of the dubious central Governments who operate in the areas most afflicted by HIV/AIDS?

We have had many exchanges in the House about the difficulties that corruption causes for Governments who want to help the poorest people in their countries. That is why we have a considerable number of safeguards to help to ensure that our money is spent effectively and goes where it is needed, and to help developing countries to build up their own defences against corruption. I agree that we need to continue to do more in that area.
The hon. Gentleman is also right to say that we must do more to promote education, especially girls' education and access to primary education more generally. That is one of the reasons why my right hon. Friends the Secretary of State and the Prime Minister have made the commitment to an £8.5 billion investment over the next 10 years from the UK to seek to achieve those objectives.

DFID has committed to spend at least £150 million over the three years from 2005-08 on programmes to meet the needs of children affected by AIDS. When the commitment was made in 2004 it was expected that at least £44 million of this would be delivered through UNICEF. The methodology to track this expenditure is under review.In 2006 the UK was the second largest donor to UNICEF, providing £105 million. This included £19 million of core financial support and £5 million to increase the coverage, quality and coordination of the global response for children affected by HIV and AIDS. Both of these elements contribute directly to supporting UNICEF's strategic goals, which include reducing the number of paediatric HIV infections by at least 40 per cent. and ensuring that at least 80 per cent. of HIV positive pregnant women receive antiretroviral therapy to prevent mother to child transmission (PMTCT) of HIV.DFID is also intensifying action on PMTCT at country level with a range of partners, including UNICEF. For example in Zimbabwe DFID has initiated a £25 million project specifically to strengthen actions to prevent, diagnose and treat HIV infections in mothers and babies. In Zambia, we support UNICEF's expansion of PMTCT services.

DFID is working with a wide range of partners, including the World Health Organisation, developing country governments, pharmaceutical companies and non-governmental organisation on the development of the Medicines Transparency alliance (MeTA).MeTA seeks to increase transparency over vital information on the price, quality and availability of essential medicines in developing countries so as to help tackle high prices, challenge corruption and address inefficiency. One third of the world’s population lacks access to essential medicines. The prices patients pay are often much higher than the price agreed at procurement, with cumulative mark-ups of 300 per cent. not being uncommon as medicines move from procurement through storage, supply, distribution and ultimately to health centres and pharmacies. In addition, up to a third of medicines on the market in developing countries are fakes.Procurement and supply systems are generally opaque, meaning excessive price mark-ups, corruption and inefficiency can go unchallenged. Most people have little idea about what medicines should cost, or what a good quality medicine looks like.MeTA seeks to change this by securing high level political commitment to increased transparency and accountability over medicines procurement and supply in countries that choose to take part, and by bringing the right people around the table—government departments, civil society organisations, companies and others—to agree ways of disclosing information on the price, the quality and the availability of essential medicines into the public arena, with the aim of ultimately reducing the cost, improving the quality and increasing the availability of medicines.MeTA builds on the success of the extractive industries transparency initiative (EITI) by applying principles of transparency and accountability to medicines procurement and supply. Application of EITI principles to other sectors was supported by G8 countries at the recent Heiligendamm summit, and their specific application to the health sector was a commitment in the 2006 International Development White Paper.A major stakeholder meeting for MeTA was held on 18 April, attended by all relevant groups and addressed by my right hon. Friend the Secretary of State for International Development. An official launch will take place in the summer, with the launch of pilots in around five countries to develop a model approach to increased transparency and accountability in medicines procurement and supply.

The UK Government are not funding any research into the development of new oral anti-retroviral medication but is funding research into the development of microbicides (vaginal medication) against HIV and AIDS. The International Partnership for Microbicides (IPM) has received £8.7 million between 2002-08 and the Microbicide Development Programme (MDP), a joint programme between the UK Medical Research Council and Imperial College London, has received £39.8 million between 2001-09.The UK Government are providing £5.6 million for two clinical trials that are developing simplified treatment protocols to use existing oral anti-retroviral therapy in Africa. Both trials are coordinated by the Medical Research Council clinical trials unit—DART (ART for adults 2002-07) and ARROW (ART for children 2006-11).

It is difficult to quantify the specific UK spend on medicines since 1997 due to the rapid changes over this period in how DFID finances health. Increasingly DFID contributes to pooled funds with other partners or provides untied funds into the health sector budget or contributes to the general budget of a country. The procurement of medicines is carried out directly by recipient governments or by the recipients of UK funds.The UK Government are a major donor to the Global Fund to fight AIDS, TB and Malaria (GFATM) and has committed £359 million through to 2008. Almost half of the GFATM funds are used to procure drugs and medical supplies. The UK also provides substantial support to other global health initiatives such as the Global Alliance for Vaccines and Immunisation (GAVI), Roll Back Malaria and Stop TB, and to UN agencies such as the World Health Organisation (WHO), UNICEF and the United Nations Population Fund (UNFPA), all of which procure medicines and/or vaccines.In September 2005, the UK launched, with other donors, the International Financing Facility for Immunisation (IFFIm). This will provide $4 billion over a 10-year period to support the purchase of vaccines by developing countries and the strengthening of immunisation systems.The United Kingdom (UK) Government are committed to improving access to essential medicines in developing countries. In general, DFID spends money on efforts to, strengthen national medicine systems; ensuring affordable pricing and sustainable financing, reliable health services and the rational selection and use of drugs, rather than buying medicines specifically.Since 1997, DFID has spent over £1.5 billion to support the development of health systems in poor countries. Specific initiatives related to medicines include providing £39 million to support the Nigeria Partnership for Transforming Health Systems and £3.6 million to the South African Drug Action programme which has increased the number of health care centres that use approved lists of essential medicines. DFID has provided support to the WHO to appoint national professional officers for essential medicines in 11 African countries, to provide technical assistance on supply, management and rational use of medicines.

The current prevalence of TB in Yemen is 1.5 per 1,00 people. The Government of Yemen runs a National Tuberculosis Programme aiming to eliminate TB in the country.The UK has committed £59 million until 2008 to the Global Fund to Fight AIDS, TB and Malaria. Since January 2002 the fund has approved US$735 million for TB control, and $6.1 million is being spent in Yemen between 2005 and 2010.DFID has committed £ million, between 2005 and 2008, to the global Stop TB Partnership which has provided support to good quality TB drug access in Yemen. The UK also provides core funding to the World Health Organisation (£2.5 million in 2005–06) which among other things, works to fight TB in Yemen

The Global Fund to Fight AIDS, Tuberculosis and Malaria does not currently collect this data.The Global Fund's procurement policy specifies that resources may be used to procure any medicines that appear in national or the World Health Organization standard treatment guidelines (STGs) or essential medicines lists (EMLs). Unlisted products may be procured only if the principal recipient of the grant states a specific rationale for doing so in its proposal to the Global Fund.

DFID has been working with the international pharmaceutical industry in a number of ways to increase access to medicines, including for the treatment of HIV and AIDS, and for children.In March 2005 DFID, the Department of Trade and Industry and the Department of Health, launched a framework encouraging ‘good practice' in the pharmaceutical industry to increase access to medicines. The framework contained a number of recommendations on how companies could increase access to medicines in developing countries, including through making medicines more affordable and through more research on diseases affecting developing countries. The framework encourages investment into new medicines for children among other areas.The UK also provides support to increase research into treatments and vaccines for diseases affecting developing countries, including for HIV and AIDS. This includes tax credits and direct support to product development public private partnerships. DFID has funded research on the use of existing medicines in developing country settings, such as a trial of co-trimoxazole that significantly reduced mortality among children living with HIV.DFID will continue to ensure the needs of children and other vulnerable groups are addressed as we work towards scaling up to universal access to treatment by 2010, as agreed by the G8 and by the millennium review summit. The Joint United Nations Programme on HIV/AIDS (UNAIDS), has established a Global Steering Committee, which the UK will co-chair. Plans will be based on the principle of country ownership—recognising the need to work through existing national structures and processes and focus on mobilising countries to scale up in line with the 2010 target.In addition the Global Steering Committee will encourage countries to identify obstacles and develop solutions, including addressing the development of appropriate treatments for key groups, such as children.

Taking Action, the UK's strategy for tackling HIV and AIDS in the developing world, published in 2004, states that, our support for children affected by AIDS is reflected in all of DFID's country assistance plans in all affected countries. This comprehensive approach includes prevention, care and treatment as well as recognising the impact AIDS has on society, including increasing orphan rates. The UK will continue to advocate for comprehensive AIDS programmes that meet the needs of children as well as adults.The WHO reports¹ that access to HIV care and anti-retroviral therapy has not been extended widely to children. However, data is incomplete and the UK has urged the WHO, the Joint United Nations Programme on HIV/AIDS (UNAIDS) and others to track the extent to which treatment and care programmes are meeting the needs of the poor, women and children.The Global Partners Forum on Children Affected by HIV and AIDS, which the UK is hosting in early February, will explore the current obstacles to treatment of children with HIV and AIDS, as well as other services and needs. In addition, the Global Steering Committee on Scaling up Towards Universal Access, which in turn is due to report to the UN General Assembly High Level meeting in June 2006, is considering the obstacles to scaling up access to treatment for children, including the lack of appropriate and effective diagnostic tools and treatments.Following publication of the DFID-funded Medical Research Council research findings, the WHO, UNAIDS and UNICEF issued a joint statement in November 2004 on the use of cotrimoxazole as a prophylaxis in HIV exposed and HIV affected children.The joint statement advocates that prophylactic dosing with Cotrimoxazole for HIV infected children with any sign or symptoms suggestive of HIV is a key intervention that should be offered as part of a basic package of care to reduce morbidity and mortality.The document further states that Cotrimoxazole prophylaxis is also a crucial potentially life saving intervention that should be given to all HIV exposed children born to HIV- infected mothers, in settings where HIV infection status cannot be reliably confirmed in the first 18 months of life.Following the joint statement, Cotrimoxazole is now universally accepted as a standard treatment package for all children exposed to HIV and on treatment and it is advocated for all children with HIV, regardless of whether they are receiving treatment.As a follow on to the Cotrimoxazole study DFID is providing funding support for a further trial through the Medical Research Council to examine the best use of anti-retrovirals for children."¹Progress on Global Access to HIV Antiretroviral Therapy—an update on ““3 by 5”” June 2005"

Taking Action, the UK's strategy for tackling HIV and AIDS in the developing world, published in 2004, states that, our support for children affected by AIDS is reflected in all of DFID's country assistance plans in all affected countries.
