Mr. Jim Cunningham: To ask the Chancellor of the Exchequer what research he has commissioned on cancer survival rates in the United Kingdom compared with other EU countries. [133894] Ruth Kelly :[holding answer 27 October 2003]: The information requested falls within the responsibility of the National Statistician, who has been asked to reply. Letter from Colin Mowl to Mr. Jim Cunningham, dated 17 November 2003: The National Statistician has been asked to reply to your recent question concerning what research the Chancellor of the Exchequer has commissioned on cancer survival rates in the United Kingdom compared with other EU countries. I am replying in his absence. (133894) Comparable statistics on cancer survival in the UK and most other EU countries have been produced in three studies funded by the European Commission (the EUROCARE studies). Cancer registries in the UK, including staff from the National Registry at ONS, have been involved in these studies since they began in 1989. The results indicated that survival was generally highest in Sweden, the Netherlands, France and Switzerland, and lowest in Estonia, Poland, Slovakia and Slovenia. Survival in the UK and Denmark was lower than in other western European countries for common tumours such as those of lung, breast, stomach, large bowel, prostate and kidney. However, international differences in survival were not large for tumours amenable to cytotoxic therapy, such as testicular cancer and Hodgkin's disease. Government plans for funding cancer research were published in the NHS Cancer Plan in September 2000; copies of which are available in the Library. The Plan acknowledged that "survival rates for many of the major cancers lag behind the rest of Europe".

Letter from Len Cook to Mr. Jim Cousins, dated 24 February 2002: As National Statistician, I have been asked to reply to your recent question concerning what steps are being taken to improve, extend and update statistics for five year survival rates for common cancers; and what programme of development work in such health statistics is underway. (99099) The Office for National Statistics is currently collaborating with the London School of Hygiene and Tropical Medicine to produce survival statistics for adult patients diagnosed with the 20 commonest cancers in England and Wales during 1991-1999 and followed up to the end of 2001. These account for almost 90% of adult cancers. The work will include estimates of the socio- economic gradients in cancer survival. Survival analyses based on complete follow up of patients for five years from diagnosis will be possible for those diagnosed up to 1996. As follow up is not yet available beyond 2001, a recently developed statistical method, "period" survival, will be used to estimate the five-year survival for patients diagnosed up to 1999. The results, which will be published later this year, will provide robust and up-to-date evidence on changes in survival during the 1990s.

[80046] The most recent available mortality data are for the calendar year 2001. Figures for each year from 1997 to 2001 are given in the table below. Notes: 1 The cause of death was defined using the International Classification of Diseases, Ninth Revision (ICD-9) code 185 for the years 1997 to 2000, and for the year 2001, the International Classification of Diseases, Tenth Revision (ICD-10) code C61. 2 Figures are for deaths occurring in each calendar year. 3 The introduction of ICD-10 for coding cause of death in 2001 means that data for prostate cancer in this year are not comparable with data for earlier years. The introduction of ICD- 10 has led to an apparent increase of 4 per cent. in the number of deaths coded to prostate cancer. The figures should therefore be interpreted with caution. The effect of the change in classification in 2001 is described in a report published in May 2002i

Letter from Len Cook to Mr. David Heath, dated 15 October 2002: As National Statistician I have been asked to reply to your question concerning the requirements of medical practitioners completing death certificates to include illnesses for which there is medical evidence rather than clinical suspicion. (73420) Medical practitioners are required to certify the cause of death for patients under their care. Current legislation requires them to ``sign the certificate in the prescribed form stating to the best of his/her knowledge and belief the cause of death''. Guidance issued to doctors asks that they describe the cause of death as fully and accurately as possible. They should describe the sequence of diseases or injuries which led to the death, and other conditions which may have contributed to it. These guidance notes follow the recommendations of the World Health Organisation. There is considerable interest in death certification at present. The Fundamental Review of the Coroner Service has recently published a consultation document on possible changes to the way in which deaths should be investigated and certified. This can be found at http://www.coronersreview.org.uk/, or a paper copy requested from The Review of Coroner Services, 100 Pall Mall, London SW1Y 5HP. The closing date for responses is 22 November 2002. In addition, the Office for National Statistics will be consulting on modernisation of the civil registration system during 2003. Any future changes to death registration will also need to consider the recommendations that will come out of the Shipman Inquiry.

Letter from Len Cook to Dr. Ian Gibson, dated 20 June 2002: As National Statistician, I have been asked to reply on what the average life expectancy is of a myeloma patient. (62688) Life expectancy is generally not estimated for cancer patients, because death rates in any age group would be much more heavily influenced by age at diagnosis than by survival from the disease. A better indicator is the proportion of patients who are still alive at various period of time after diagnosis. This is used almost universally in the assessment of cancer control programmes. The crude survival rate is simply the proportion of the original group of patients alive at the specified time since diagnosis, i.e. it takes into account both deaths from the cancer concerned and deaths from all other causes. The latest available information on crude survival for patients diagnosed with multiple myeloma in England and Wales is given detailed in the table.
